Comox Valley Seniors Community Connector

United Way BC’s Healthy Aging program is supporting new Social Prescribing programs in communities across the province including the Comox Valley. Social prescribing connects older adults to community-based services, activities, and supports, that address the social determinants of health.

We are hiring a Seniors Community Connector to play this integral role in bridging the gap between healthcare and social care. This role includes a diverse range of responsibilities from conducting assessments and co-developing personalized care plans, to providing practical support to older adults and their families, to establishing, and maintaining relationships with people and organizations in the community offering supports to older adults.

Who is a Community Connector? A trusted relationship-builder who is effective at supporting older adults to strengthen their health and wellbeing by connecting them to community-based supports, activities, and networks.

A Community Connector;

• builds trust and practices active listening,

• applies strengths-based, trauma-informed, and culturally humble approaches,

• identifies and mobilizes community assets,

• works independently with strong time management and organization,

• maintains appropriate professional boundaries and practices self-care,

• communicates effectively across diverse settings,

• navigates basic technology for documentation and communication, and

• adapts creatively to emerging needs and opportunities.

This position averages 30 hours per week with an annual salary of $51,420. and is eligible for the Society’s benefit plan. This position is based in the Comox Valley and will start immediately. The job description follows.

Comox Valley Seniors Community Connector

Job Description

 A Seniors Community Connector plays an integral role in bridging the gap between healthcare and social care. As a Social Prescribing professional, you partner with community organizations to provide non-medical support to improve the overall health and well-being of older adults in the Comox Valley. 

 The Seniors Community Connector works with older adults who are referred from healthcare professionals and community partners to connect with community supports and services through referrals, applications, advocacy, and introductions. This role includes a diverse range of responsibilities – from conducting assessments and co-developing personalized care plans, to providing practical support to older adults and their families, to establishing, and maintaining relationships with people and organizations in the community offering supports to older adults. Your experience, training and interpersonal skills help you to make meaningful connections as you discern their unique needs. Through your work with local agencies and professionals, you will make a positive impact on the lives of older adults in the Comox Valley and our community. 

 The goal of this position is to assist older adults to age safely in the right place by providing them with the social connections they require and build and maintain partnerships and connections of community-based seniors services in the Comox Valley.

 Roles and Responsibilities:

 Community Development

·        Build and maintain networks within community and strengthen relationships withing the community-based senior serving sector.

·        Use an asset-based community development approach to identify and mobilize individuals and organizations providing and supporting older adults.

·        Engages and participates in learning opportunities such as in community of practices, planning tables or networking events.

 Assessment and management of referrals

·        Connects, liaises, and establishes partnerships with local healthcare professionals to create and maintain referral pathways.

·        Maintains an active caseload of seniors with short-term needs through referrals from healthcare professionals and community agencies.

·        Prioritizes referrals to meet individual participant’s needs.

·        Understand hospital discharge procedures to assist with supporting seniors transitioning back home following discharge.

·        Implementing safety precautions when visiting seniors in the community, including their personal residence.

·        Completes intake process to assess strengths, needs, abilities, and risks using motivational interviewing techniques such as active listening, conflict resolution, and observing behaviour.

·        Use various assessment tools to determine challenges, needs and risks related to healthy aging, and develop routines, structures, and resource referrals to reduce risk of frailty.

·        Interpret participants’ complex physical requirements and social needs.

·        Maintains a high level of confidentiality in all matters related to clients and community partners.

 Wellness Plan development

·        Supports seniors to access appropriate range of activities and suitable community resources by developing individual wellness plans.

·        Using a “what matters to you” approach refers seniors to community-based services, observes and assesses the participant’s engagement with resources including (example: emotional, psychological, and functional status), and modifies activities to meet the participant’s changing needs.

·        Provides information regarding appropriate community resources to socially support the participant and their families.

·        Effectively collaborates within our organization and other Comox Valley Seniors Services to provide multidisciplinary care for the best interest of the senior.

·        Assists with connection to a primary care provider.

·        Engages and participates in educational training for seniors on topics such as healthy aging and other relevant topics.

 Documentation of referrals and evaluation

·        Documents participant’s interactions, wellness plans, reports, and other administrative duties as required.

·        Provision of follow-up notes to continuing community health care provider if requested. 

·        Participates in evaluation of programs including collection of participant data, reporting at regular intervals, and attending communities of practices.

 Qualifications:

·        Bachelor’s degree in social service or related Human Services field or a combination of relevant education and experience.

·        Strong knowledge of seniors’ issues and challenges related to healthy aging.  

·        Experience working with seniors and diverse populations from different cultures and socio- economic backgrounds.  

·        Knowledge of the community resources, programs and services.  

·        Minimum two (2) years recent related experience.

 Job Skills and Abilities:

  • Demonstrated ability to connect with and support seniors.

  • Demonstrated skills in the areas of crisis intervention and conflict resolution.

  • Demonstrated ability to recruit and supervise volunteers.

  • Strong collaborative skills and proven ability to establish and maintain effective working relationships with all internal and external contacts.  

  • Excellent written and oral communication skills and ability to clearly explain instruction to others.

  • Ability to work independently with strong time management and organizational skills.

  • Strong computer skills.

  • Valid driver’s license and reliable vehicle with appropriate insurance.

  • Provide a satisfactory BC Ministry of Justice Criminal Record Check, will be completed by Hornby Denman Health at time of hiring.

Seniors Day Program Coordinator - Hornby Island

We are hiring a Coordinator for our Elder Connections day program on Hornby Island. This program supports older adults to remain independent in their homes and active in their communities. Part of a provincial Healthy Aging program, this program aims to promote seniors’ social well-being, delay use of health services and provide respite to caregivers. The day program is held once a week and includes proactive outreach to check-in on participant well-being and arrange transportation if required, a mix of physical, educational, creative and social activities, and a nutritious meal at a low cost to participants. An additional day of programming will be added this year.

The Coordinator is responsible for the day-to-day activities of the program including:

●   planning, coordinating and supervising services,

●   working with the Seniors Program Manager to train and supervise staff, volunteers and contractors,

●   collecting and tracking data required for reporting, and

●   participating in the provincial community of practice and quality assurance activities in collaboration with Healthy Aging BC and UBC.

The successful applicant will be organized, personable, reliable, and a supportive team leader. They will have training in therapeutic recreation and/or geriatric services, and/or a related field, and/or equivalent related experience.

This position is part time, approximately 16-18 hours per week and it is eligible for the Society’s benefit plan. Hourly starting rate is $26.25/hour. Position to start immediately. Full job description below.

Job Description Seniors Day Program Coordinator

In accordance with the established vision and values of the organization, the Seniors Day Program Coordinators report to and work closely with the Program Manager to implement this weekly program. This program supports older adults to remain independent in their homes and active in their communities. Part of a provincial Healthy Aging program, this program aims to promote seniors’ social well-being, delay use of health services and provide respite to caregivers. The program is one or two days per week on each island. The program includes proactive outreach to check in on participant well-being and arrange transportation if required, a mix of physical, educational, creative and social activities, and a nutritious meal at a low cost to participants. There is one Program Coordinator for each island and while each focuses on implementation of the program in that community, they also regularly collaborate on program delivery, and cover for each other during absences.

 KEY DUTIES:

●   Assist with the design and delivery of program activities,

●   Recruit, asses, and monitor the needs of program participants, providing outreach as required to connect with vulnerable and isolated seniors,

●    Assist with recruiting, screening, training and supervising of other program staff, contractors and volunteers,

●    Assist with creating, monitoring and maintaining appropriate safety plans and risk management protocols,

●    Assist with promoting the program and engaging the support and involvement of health service providers, local organizations and the community,

●    Assist with maintaining the program database and appropriate records for evaluation as required internally and by funding organizations,

●    Participate in knowledge and information sharing through the provincial community of practice,

●    Manage program facilities, equipment and supplies,

●    Carry out other duties as assigned by the Program Manager.

Qualifications:

Typical Education, Training, and Experience

●     Diploma or degree in human services or related field, or a combination of education and related work experience,

●     Demonstrated success organizing and leading group activities,

●     Demonstrated success supervising staff and volunteers,

●   Proficient computer skills including basic spreadsheet and word processing, online forms, and reporting,

●     Experience and interest in working with a diverse community including different cultures and socio-economic backgrounds,

●     Valid Class V BC Driver’s License,

●     Certificates in CPR, First Aid, and Food Safe - can be completed once hired

 Typical Skills and Abilities

●     Physical ability to carry out the duties of the position

●     Ability to establish and maintain rapport with participants, staff and volunteers

●     Ability to observe and recognize changes in participants

●     Ability to work independently and in cooperation with others

●     Ability to communicate effectively, both verbally and in writing, make presentations and facilitate training and group activities
